ENTRY CARDS: Each individual athlete (running or field events) along with each relay team must submit a completed 3x5 card along with this form by deadline dates listed above. There must be a card for each and every entry in each and every event.Each card must contain the following:

ATHLETE'S NAME (not necessary for relays)
SCHOOL
TIME or DISTANCE


OFFICIATING: Each team that is entered in the meet must provide one official to officiate the meet from its start to its end. A school entering both a boys and a girls team must provide two officials for the meet.
